What to expect? 

Once you have book your appointment, we will be in touch to provide a quote for the required assessment, as well as sending a service agreement for the appointment and subsequent assessment report writing time. 

If required, please have a support person with you during the appointment to assist in gathering accurate information so the therapist can begin on your assessment promptly and accurately. 

For certain assessments, the occupational therapist may be required to observe you completing an activity such as cleaning, caring for yourself etc.

Where applicable, please be prepared for observation based assessment. 

Further assessments required: 

An occupational therapist will be in touch regarding the nature of the assessment required, where possible assessments will be completed during the scheduled appointment time. If your report requires additional assessments this will be discussed with you. 

Information gathering from people in your support network may also be required to complete the assessment. This will be discussed during the appointment.

3. Choose a time that best suits you 

4. Please provide: 

- NAME OF THE PERSON THE ASSESSMENT IS FOR

- Best contact number and e-mail for the appointment 

- Date of Birth of the person the assessment is for 

- Comments: PROVIDE TYPE OF ASSESSMENT, THE PERSON WHO IS BOOKING THE ASSESSMENT (family member/support co-ordinator)
